AleraCare, a provider of ambulatory infusion treatments, has an exciting new opening for a Per Diem Infusion Nurse Practitioner for our Los Angeles metro area clinics. This is a travel (float) per diem position with a competitive hourly rate. We are headquartered in Phoenix and currently located in 8 western states with plans for expansion. The clinic hours are Monday – Friday,8:00am-5:00pm. Position Summary
The Infusion Nurse Practitioner is responsible for providing nursing services to assigned patients in accordance with the patient’s plan of care. This position reports directly to the VP of Nursing.
Responsibilities
- Responsible for the clinical record of each patient receiving nursing care.
- Assures that progress reports are made to the physician for patients receiving nursing services when the patient’s condition changes or deviations from the plan of care.
- Adheres to the facility’s policies and procedures, including patient care policies.
- Performs initial patient evaluations/assessments and develops a written plan of care.
- Regularly reassess the patient’s condition and nursing needs and update the plan of care.
- Furnishes those services requiring substantial and specialized nursing skills.
- Initiates appropriate preventive nursing procedures.
- Prepares clinical and progress notes, coordinates services, and informs the physician and other personnel of changes in the patient's condition and needs.
- Counsels the patient and family in meeting nursing and related needs.
- Demonstrates competency in providing patient care and provides care in accordance with professional standards and the state’s nurse practice act.
- Initiates case conferences and communicates the patient plan of care with team members.
- Evaluates and re-evaluates the need for services.
- Provides appropriate patient education and teaching materials.
- Participate in the Performance Improvement program as requested by the Performance Improvement Coordinator.
- Participate in committees when requested.
- Attends continuing education programs and staff meetings as appropriate to job responsibilities.
- Represents the facility positively and professionally.
- Perform other duties as assigned by supervisor.
Minimum Qualifications
- Graduate of an accredited school of nursing and current NP state license.
- At least one year of nursing experience, infusion nursing experience is preferred.
- Effective interpersonal, time management, and organizational skills.
- Strong communication skills in speaking and writing.
- Organizational skills sufficient to maintain consistently accurate records.
- Ability to recognize and direct information to the appropriate health care provider.
- Computer skills that include word processing and efficient use of the internet and e-mail.
- Current CPR card, health certificate, valid driver’s license, reliable transportation, automobile insurance.
