Position Summary:
The Clinical Nurse (RN), under the direction of the Director of Nursing and the clinical care team, serves as an integral member of the CCI nursing and primary care teams. The RN is responsible for assisting and supporting the CCI clinical team in providing high quality, direct patient care assessments, participating in continuity of patient care and helping to improve patient outcomes through care coordination and education.
As a member of the health center’s integrated care team, the Clinical Nurse (RN) fulfills the following KEY FUNCTIONS AND RESPONSIBILITIES for the provision of patient-centered, coordinated patient care:
- Performs routine nursing care and some care coordination duties to optimize patient care, upholding high quality and safety standards for CCI’s population of patients from infancy to geriatrics, which may include:
- Developing a working rapport with patients.
- Participating in clinical huddles, pre-visit planning and identifying complex care patients.
- Conducting patient appointments in collaboration with clinicians - collecting patient health history, assessing, screening, triaging, analyzing, and identifying a patient’s healthcare needs and care gaps.
- Providing appropriate medical advice and discharge instructions to patients with the support of evidence-based resources for pediatric, adult, and obstetric populations.
- Reviewing and reconciling medical and vaccine records - completing required forms and paperwork needed by patients, schools, and other healthcare entities.
- Reconciling medication history and counseling patients on the use, benefits, risks, and potential side and adverse effects of any medications prescribed and administered.
- Coordinating clinical care, transitions of care and supporting the referral process.
- Counseling and education on chronic disease management, primary prevention activities, abnormal labs and diagnostic imaging and working with care teams to determine appropriate follow-up screening and notification to local and state agencies as indicated.
- Developing care plans, providing verbal and written information in the patient’s preferred language by utilizing in-person or telephonic interpreters.
- Executing treatment plan as directed by the Director of Nursing, and/or clinician and evaluating results including providing point of care testing, vaccination and medication administration, refilling patient prescriptions and obtaining prior authorizations.
- Plans, coordinates, and/or attends assigned meetings, health fairs, events and trainings.
- Assists in quality assurance, quality improvement and workflow analysis and innovations in the nursing department.
- Assists with training new employees and ongoing training of medical assistants and other support staff.
- May directly supervise medical assistants per the direction of the nursing leadership and assist with managing the team’s schedule.
- Consistently seeks avenues for self-education to broaden knowledge base and maintain compliance with evidence-based practice.
- Uses the Electronic Medical Record to maintain patient medical records, utilizing standard language and abbreviations and following accepted medical standards for charting.
- Provides quality customer service and communicates in a HIPAA compliant, culturally sensitive and trauma-informed manner.
- Works within the policies and procedures of CCI Health Services Inc.
- Performs other duties determined as necessary by the Director of Nursing and other CCI leadership.
MINIMUM QUALIFICATIONS:
- Active RN license issued by the State of Maryland Board of Nursing or any state under the Nurse License Compact.
- Current BLS/CPR certification required.
- Written and oral fluency in English.
