Responsibilities:
- Tracks/trends referrals/admits/discharges
- Coordinates and collaborates with referring Case Managers and Discharge Planners. Cultivates and maintains relationships with referral sources
- Conducts outreach with hospitals and referral sources on a routine basis/as able
- Reviews/assesses clinical records/discharge summary prior to admission to assess appropriateness for placement
- Partners with DON/Assisted Living Administrator to review appropriateness for placement
- Fully completes admission forms on all new admissions or readmissions
- Obtains insurance authorizations for new admission(s)
- Identifies opportunities for new partnerships
Requirements:
- Minimum of 3 years experience, previous Long-Term Care experience preferred
- High School Graduate or equivalent required
