Dedicated to creating opportunities to transform lives within the City of Paterson's Great Falls Neighborhood, New Jersey Community Development Corporation (NJCDC) is a private, non-profit community development and social service agency focusing on education, youth development, affordable and supportive housing, and community building initiatives.
Founded in 1994 by Bob Guarasci with one other employee, NJCDC has grown into a $19 million agency, employing 150 individuals that serve over 4,000 children, teens, and families daily.
The Spruce Terrace Apartments provide safe housing and community support services to eight formerly homeless individuals with mental health diagnoses. One of several of NJCDC's housing programs focusing on the increased challenges for homeless individuals and individuals with mental health conditions, the residents benefit from onsite staff supporting them to live independently.
Qualifications:
- Master’s Degree from an accredited college or university in Social Work, Rehabilitation Counseling, or other related behavioral health or counseling program.
- *** Possesses credentials as a Licensed Clinical Social Worker, Licensed Social Worker, Licensed Rehabilitation Counselor, Licensed Professional Counselor or Licensed Marriage and Family Therapist. ***
- Three years of experience in a community behavioral health setting.
- Demonstrated experience working in wellness and recovery programs.
- Excellent organizational skills. Self-starter. Ability to work independently. Ability to multi-task.
- Demonstrated leadership capacity and sound judgment.
- Computer literacy with proficiency in MS Office products (Word, Excel, PowerPoint).
- Excellent interpersonal, verbal, and written communication skills.
- Ability to work effectively as a member of a team.
- Ability and enthusiasm for working with staff at all levels of the organization in a collaborative team.
- Valid, clean New Jersey driver’s license.

Essential Duties and Responsibilities:
- Provides administrative supervision to the entire program and clinical supervision of the master’s level staff member.
- Performs comprehensive rehabilitation needs assessment.
- Contributes to the development, implementation, monitoring and updating of the individualized rehabilitation plan.
- Performs therapeutic rehabilitative skill development; offers services to meet consumer’s needs.
- Performs crisis intervention.
- Coordinates and manages services.
- Ensures overall performance of program in meeting identified outcomes, including expectation of a program culture that is inspiring, nurturing and supportive.
- Prepares program start-up documentation including, but not limited to, the program policy and procedure manual and updates documents as appropriate.
- Hires, trains and supervises and evaluates program staff.
- Conducts new staff orientation and training. Ensures that all program staff complete required training modules within the allotted time frames.
- Provides ongoing staff orientation and supervision, including meeting one on one with program staff at least monthly to provide formal feedback and to discuss outcomes and progress.
- Completes annual performance appraisal for each staff member, including quarterly performance interviews.
- Establishes the bi-weekly staff schedule. Reviews and approves bi-weekly staff timesheets, vacation requests and submits to Director of Residential Operations.
- Serves on an on-call (24-hour) capacity.
- Coordinates services and schedules of program consultants.
- Assumes property management function as directed by supervisor.
- Coordinates the planning for new consumers to move into residence, including reading records and completing evaluations to determine appropriateness of potential program consumers as well as ensuring that all admissions records are in place prior to the move-in-date.
- Coordinates the annual Individual Service Plan process for each resident. Reviews plans on a quarterly basis and makes revisions as needed. Tracks progress related to consumers’ ISP goals and objectives.
- Produces the monthly outcomes report and trains all program staff in documenting and utilizing the tracking system.
- Establishes, supervises and evaluates the methods used to teach consumers new skills. Directs program staff to provide services in accordance with established methods.
- Creates a monthly training schedule that addresses specific goals and objectives flowing from each resident's Service Plans (and ISP). Distributes this schedule to program staff and ensures adherence to the schedule.
- Coordinates clinical and supportive services for consumers. Determines how program staff will provide assistance to each resident and oversees staff's provision of the direct assistance.
- Maintains communication with each resident’s doctor and other health/mental health care providers. Family members, guardians and any other significant person involved in the resident’s life.
- Ensures the timely and thorough completion of program documentation as required by state licensing standards, agency policy and procedures and quality assurance guidelines.
- Participates in the review, at least quarterly of the House Rules and updates the same as needed.
- Reviews the daily logs and incident reports at least once every 72 hours to identify any errors, problems, trends or issues which require intervention.
- Communicates matters regarding the facility and consumers to the Director of Residential Operations in a timely fashion. This includes the care and condition of consumers, staff performance, the physical condition of the facility and program vehicle and any other matters that are not routine in nature.
- Maintains full confidentiality of consumers’ information in compliance with the Health Insurance Portability and Accountability Act (HIPPA). Ensures that confidential records are secure, complete, accurate and easily accessible to individual consumers. Ensures that a HIPPA consent form is on file for each resident and that it is updated annually.
- Ensures that all program staff is aware of emergency incident report protocol.
- Facilitates core staff meetings on a monthly basis. Participates in agency-wide staff meetings, department-wide meetings and any other meetings that may be called by NJCDC management.
- Maintains the physical condition of the residence. Ensures that the residence and grounds are clean. Performs housekeeping duties as necessary. Conducts a weekly full inspection (walk-through) of the facility and grounds reporting any item requiring repair to the Director of Facilities immediately.
- Monitors program performance outcomes and completes required reports.
- Provides emergency coverage during staff shortages at times when management so directs.
- Coordinates outreach efforts to identify individuals in need of housing, establishes referral process and conducts intake interviews to determine appropriateness.
- Other duties as assigned.
Annual salary is $80-$90k depending on experience.
