Northwell Health’s Medicine Service Line, Division of General Internal Medicine and Northwell Health’s Cancer Institute are partnering together to hire a board eligible/board certified Internal Medicine Physician to join our Cancer Survivorship Program at the Zuckerberg Cancer Center in New Hyde Park, NY. This program seeks to deliver a new model of care where survivorship clinicians begin working with oncology to manage patient toxicities (i.e., fatigue, lymphedema, neuropathy, etc.) and co-morbidities related to disease and treatment. This physician will work as part of a multi-disciplinary team and enable timely referrals to sub-specialists and/or programs to optimize patients’ long-term health and well-being. This is an exciting opportunity to help launch our new care model to transform the health of our patients.
Zuckerberg Cancer Center is located at the Center for Advanced Medicine, a nationally recognized cancer care facility offering state-of-the-art technology, leading-edge clinical trials, and a full range of support programs. Our team is composed of a large, dynamic, and diverse group of clinicians, educators, and researchers from multiple professions eager to take on the challenge of patient centered longitudinal, and comprehensive cancer care services. Our center is a unique facility that provides exceptional care for virtually every type of cancer with a wide range of coordinated treatments, including Ambulatory hematology, Leading-edge chemotherapy, Immunotherapy, Hormone therapy, and Targeted therapy. Patients are offered clinical trials as the standard of our care.
Applicants should be enthusiastic about working collaboratively across specialties to co-manage patients, comfortable working in and providing feedback on an iterative process to develop a new care model, have a passion for learning and teaching, and interested in using technology effectively to deliver high quality care. Expertise in cancer survivorship is preferred.
