Why This Role is Important to Us:The Manager, Clinical Care Partnerships will play a vital role in the success of the entire Care Partnership organization at CCA. The main expectation of the Clinical Manager is to be accountable for the performance of the team, by ensuring that all key performance metrics are met as well as the team's daily operations, including the member experience, care management, care coordination, and care delivery. The Clinical Manager will ensure that the team meets clinical and health plan related KPI's (Key Performance Indicators) which are set by clinical leadership, regulators, and the business. Commonwealth Care Alliance's (CCA) Care Partner Team is primarily responsible for providing longitudinal care coordination, care management, and care delivery to a dedicated panel of CCA members, a group of individuals with significant medical, behavioral, and social complexities.
As a keystone of the Clinical Group leadership team, the Manager, Clinical Care Partnerships participates in the development of tactics and is responsible for the execution of strategies to meet objectives. The Manager, CCP is engaged in the hiring and onboarding, performance management and staff enablement to support and improve care delivery, care management, and clinical operations in alignment with the strategic goals of CCA.
What You'll Be Doing:This position covers Allston, Arlington, Auburndale, Belmont, Billerica, Brighton, Burlington, Chelmsford, Chestnut Hill, Dunstable, Lexington, Lowell, Medford, Newton, North Billerica, Reading, South Chelmsford, Stoneham, Tyngsborough, Waltham, Watertown, Winchester and Woburn.
- Responsible for leading a care management and care coordination team.
- Monitors and reports performance related key performance indicators (KPIs) to ensure clear visibility into the team's operational and outcome performance excellence, regulatory compliance and achievement of performance goals.
- Leads with a strong emphasis on data management.
- Cultivates and maintains collaborative relationships.
- Create a culture of curiosity, problem solving, and continuous learning. Encourage Root Cause Analysis (RCA) and 5-Why's when appropriate to fully understand how to overcome challenges.
- Lead by example with strong motivational interviewing, time management, emotional intelligence, conflict management, and change management skills.
- Hold direct reports accountable for performance, developing, coaching and/or implementing disciplinary actions as needed.
- Participate in hiring process for team.
- Participate in workflow development and/or enhancement initiatives as requested.
What We're Looking For:Qualifications:
- Associate's Degree or 2+ years of applicable experience required, Bachelor's Degree or equivalent experience preferred.
- Valid driver's license with ability to travel to CCA sites across the state of Massachusetts and to visits in a home, residential, or provider-based setting required.
- 2+ years with a health plan, medical group or health-related facility such as a hospital or outpatient center required.
- Experience with cross-functional collaborative process improvement required.
- 5+ years of related clinical experience preferred.
- 1+ years of leadership experience preferred.
- Strong leadership and team-building skills.
- Ability to function independently and effectively and as part of an interdisciplinary team. Ability to manage and lead significant change.
- Excellent communication skills.
- Experience using motivational interviewing techniques in a professional setting.
- Ability to teach, mentor, and coach.
- Ability to work in a fast-paced environment with changing priorities while maintaining - and fostering in staff and partners - a positive and productive attitude, striving for excellence and continuous quality improvement.
- Must be familiar with using a laptop computer as well as Windows processing systems and have knowledge of Microsoft Office, specifically Word, Excel, and Outlook.
- Able to support teams with utilization of telehealth technologies (video, chat, etc.), when appropriate, as an approved modality for a variety of clinical care and care management services.
- English required, Bilingual preferred.
