Description
We are looking for a passionate and dedicated individual to lead and mentor our interdisciplinary geriatric care team. Our team is committed to providing comprehensive, concierge-level medical care and social support to frail, poly-chronic elderly Medicare Advantage patients in the comfort of their homes. The ideal candidate will collaborate with various clinicians and administrative teams to deliver exceptional clinical and operational outcomes to patients and their families.
Responsibilities
- Clinical Leadership: Oversee and guide the clinical “Community Team” dedicated to a specific patient community population. Manage, mentor, and deliver primary, urgent, and acute medical care to an assigned population of patients.
- Collaboration: Work closely with physicians, nurse practitioners, social workers, paramedics, and other healthcare professionals to ensure continuous, high-quality care across the entire care continuum – from chronic and urgent care in the home to hospital, skilled nursing facility, assisted living, palliative care, and end-of-life care.
- Community Liaison: Serve as an ambassador, building and maintaining relationships with community physicians, hospitals, and other medical facilities, including skilled nursing facilities (SNFs).
- Strategic Planning: Understand the local and national healthcare landscape, prioritize opportunities, and act on promising initiatives to improve clinical, cost, partnership, and quality metrics.
- Relationship Building: Foster key relationships within the community network, including hospitals and SNFs. Identify and collaborate with hospitalists and SNF groups to achieve mutual patient outcomes and create contractual relationships when needed.
- Organizational Growth: Partner with leadership to grow the organization, innovate the care model, and improve execution. Develop constructive cross-functional and matrix relationships with business partners to enable the model.
- Clinical Coverage: Be willing to float within the community physician role as needed.
Qualifications
- Doctor of Medicine (MD) or Doctor of Osteopathy (DO)
- Current and valid state medical license
- Board Certified in Internal Medicine (IM), Family Medicine (FM), or Geriatrics
- Basic Life Support (BLS) certified
- Valid driver’s license, access to a car, willingness to travel to patient homes/locations, and active auto insurance policy
- 3+ years of experience managing a P&L (preferred)
- Strong computer skills, including proficiency in Word, Excel, PowerPoint, and EHR medical documentation
- Excellent verbal, written, presentation, and interpersonal communication skills
- Exceptional bedside manner and time management skills
Supervisory Responsibility
This position has direct management authority.
