Description
Will work with interdisciplinary teams at Providence ElderPlace to provide pharmaceutical care services to geriatric participants. The pharmacist will provide drug therapy management and consultative services which will include a review of participant's drug profiles, working with prescribers to optimize drug regimen, monitoring quality of pharmacy practice, and providing inservice education to Interdisciplinary Teams. The clinical pharmacist will also serve as ElderPlace's liaison to the dispensing pharmacy. Efforts will focus on optimizing drug therapy outcomes, improving drug utilization, improving patient compliance, preventing adverse drug events, and acting as a drug therapy resource to physicians and other members of the health care team.

Required qualifications:
- Graduate from an accredited college of pharmacy.
- PharmD.
- upon hire: Oregon Pharmacist License.
Preferred qualifications:
- within 2 years of hire: BPS Board Certified Pharmacist.
- Specialty residency in geriatric care or experience in geriatric care drug therapy management.

About the Team
Providence Home and Community Care (HCC) is a service line within Providence with over 8,000 caregivers and 2,000 volunteers serving nearly 200,000 people a year, in their homes, in clinics and in a variety of congregate living ministries across our seven-state footprint.
HCC provides a full range of services and support for people of all ages, especially those facing chronic or life-limiting illnesses. Our core competencies include restorative care, longitudinal care and care through the end of life.
