Quality Improvement & Health Equity Program Manager
Reporting to the Manager, Quality Improvement, the Quality Improvement & Health Equity (QIHE) Program Manager is responsible for Quality Improvement and Health Equity programs and oversight. You will connect health equity to QI and care experience. Primary responsibilities include health equity oversight, regulatory and accreditation reporting, including the Health Equity Quality Measure Set, and leading SFHP's cross-departmental health equity efforts, programs and interventions.
Salary: $75,000 to $90,000
WHAT YOU WILL DO:
- Coordinate major Quality & Health Equity priorities by supporting measure identification and prioritization based on regulatory requirements and NCQA Health Plan and Health Equity standards.
- Collaborate with the Chief Health Equity Officer and Quality & Population Health Management staff in co-producing the annual Quality Improvement & Health Equity Transformation Program Description, Work Plan and Evaluation which outlines and evaluates SFHP’s Health Equity efforts.
- Work with the Quality Data Analytics team to ensure stratified reporting of Health Equity Quality Measure Set and project manages required improvement projects or participates in projects that result from measures not meeting set benchmarks.
- Produce quality and health equity related materials to solicit feedback from the SFHP’s external groups including the Quality Improvement and Health Equity Committee, the Member Advisory Committee, and provider network collaborative meetings.
- Provide guidance in project teams related to Quality and Health Equity, to ensure that project work is measurable, impactful, cost-effective, and consistent with organizational goals.
- Participate in quality and health equity work plan development, completion of work, and mid-term and long-term strategic planning of Health Equity.
- Identify causes for quality improvement and health equity through measurement, collaboration, and systems analysis.
WHAT YOU WILL BRING:
- Required: Bachelor’s degree in Public Health, Business, Sociology, or related area, or equivalent relevant work experience.
- At least three years’ experience in project management or program management in health care operations or quality improvement or equivalent work experience or certifications such as Project Management Certification.
- Desired: Master’s degree in healthcare field; experience working in health care delivery system or at a health plan.
- Familiarity with the Model for Improvement, health care, managed care terminology, Medi-Cal.
