WELLBE INTRODUCTION
The WellBe care model is a provider led Advanced Geriatric Care Program focused on the quality of care of the frail, poly-chronic, and elderly Medicare Advantage patients. This population is typically underserved and very challenged with access to overall health care. To address these problems, we have elected to bring the care to the patient, instead of trying to bring the patient to the care. WellBe’s Care Model is to provide our members the entire continuum of care from chronic to urgent care in the home, hospital, skilled nursing facility, assisted living, palliative care, and to end of life care. WellBe's physician/advanced practicing clinician led geriatric care teams partner with the patient’s primary care physician to provide concierge level geriatric medical care and social support in the home as well as delivering and coordinating across the entire care continuum.
GENERAL SUMMARY
WellBe Senior Medical is building a team of passionate, resourceful and dedicated team members to focus on market based engagement. As the leader of the Community Engagement Specialists, you will be responsible for building and leading a team to enable the development and execution of innovative ideas to market the value of WellBe Senior Medical to attributed Medicare patients. Through in-person, telephonic outreach and community events, your team will build trust-based relationships with attributed patients leading to their engagement with WellBe Senior Medical. The team is also accountable for promoting WellBe Senior Medical meeting patients where they are, i.e physician offices, hospitals, sub-acute facilities, and long-term care settings, homes.
RESPONSIBILITIES
- Build and manage a high-performing team, fostering an environment of continuous learning and development.
- Provide mentorship and guidance to team members, enhancing their skills and career growth.
- Lead and support the effort to identify innovative, entrepreneurial strategies to enroll engage patients with WellBe.
- Identify and onboard Community Engagement Specialists and supervisors.
- Train and continuously upskill Community Engagement Specialists and supervisors on the WellBe value proposition and community engagement, policies, procedures and processes.
- Engage in educational interventions with Community Engagement Specialists and supervisors to support collaborative relationships across the engagement ecosystem.
- Outbound telephonic outreach to engage patients.
- Build awareness of the benefits of WellBe with attributed patients through multiple grassroots activities including health fairs, presentations at senior buildings, canvassing and activities in communities with critical mass of attributed patients.
- Use critical thinking skills to create outreach strategies that are designed to maximize patient awareness and engagement with WellBe.
- Successfully meet and/or exceed engagement monthly targets.
- Lead the daily reporting, documentation and detailed pipeline management to prioritize telephonic and field-based outreach to patients.
- Address patient concerns and schedule for an initial visit from a provider.
- Follow up with patients to ensure they keep their scheduled initial visit.
- Provide exceptional, compassionate customer service.
QUALIFICATIONS
Educational Requirements:
Required Skills and Abilities:
- 7+ years of related experience in a sales, direct marketing, community relations or customer service capacity. Prior experience in healthcare preferred.
- 10+ years management experience.
- Entrepreneurial mindset.
- Exceptional communication skills and emotional intelligence, with the desire to help patients live a meaningful life.
- Outstanding organizational and project management skills.
- Comfort with ambiguity and ability to proactively structure work and problem solve independently.
- Proven success with time management and territory organizational skills.
- Proficient in the use of Microsoft Office Word, Excel, and PowerPoint with strong written and public communication skills.
- Access to a vehicle that you can use every day in job functions.
- Familiarity with customer relationship management (CRM) systems preferred.
Supervisory Responsibility:
Travel requirements:
- Travel may be required up to 50%.
Work Conditions:
- Hybrid role with expectations of being in the office for meetings and events and in the field to conduct the responsibilities of the role.
