Overview
LifeLong Medical Care is recruiting to fill a full-time (40hr per week) Street Nurse position within the innovative Street Medicine program. The Street Nurse leads a multidisciplinary team to assess, treat, and support individuals experiencing homelessness. This position will be deployed in one of the six service areas in Northern Alameda County.
The Street Nurse role requires excellent leadership, clinical, and patient engagement skills. The Street Nurse is essential in organizing workflows for the team by prioritizing patient care based on acuity and complexity, available resources, and the diverse skill sets of the team members.
Each team spends approximately 20 hours/week conducting field-based outreach. The Street Nurse is responsible for conducting nursing assessments, triaging needs, developing care plans, and coordinating care with the medical provider.
The Street Nurse will join a 22-person program dedicated to field-based medical care for vulnerable Alameda County residents.

Responsibilities
Essential Functions:
- Provides nursing care outreach in encampments and streets.
- Implements care plans and provides urgent and primary care services.
- Collaborates in managing common health care problems in homeless populations.
- Performs timely client/patient telephone advice and triage assessment.
- Provides education and counseling on various health topics.
- Conducts initial and continued assessment of client health status.
- Administers immunizations, medications, and blood draws.
- Provides outreach, information, and referral services.
- Implements innovative interventions for high-risk individuals.
- Maintains medical records using EMR platforms.
- Communicates effectively with clients and staff.
- Participates in clinical case conferences.
Qualifications
Qualifications:
- Current and valid California Registered Nurse license and BLS.
- Proficient in Microsoft Office Suite.
- Minimum of one year experience in community health as an RN.
- Possession of a current driver's license and insurance.
- Demonstrated ability to provide services to vulnerable populations.
Job Preferences:
- Bachelor's Degree in Nursing.
- Lived experience working with people experiencing homelessness.
Physical Demands and Work Environment
The employee frequently works outside and may be required to attend evening meetings, travel, and work evenings or weekends.
Supervisory Responsibility
This position may have direct staff supervisory responsibilities.
