Medical Director of Geriatric Services Position Summary:
The Medical Director of Geriatric Services provides direct care patients in skilled nursing facilities, assisted living facilities and personal care facilities. The Medical Director of Geriatric Services supports and coordinates care with the Interdisciplinary Group (IDG) including participation in care planning meetings and performs other necessary administrative functions, including certifications, recertifications, pre-authorizations and completion of medical forms. The Geriatric Medical Director participates in education and outreach activities within the community.
Position Duties & Responsibilities:
- Conducts medical assessments and evaluations of elderly patients in facilities and homes as indicated. Develops individualized treatment plans that take into account the patient's health history, current medical conditions, medications, and other factors. Prescribes medications, order diagnostic tests, and interpret test results to determine the most appropriate course of treatment.
- Acts as facility medical director in designated facilities and provides all required oversight to care of patients ; attends required facility meetings in capacity of medical director.
- Helps reduce readmissions to the hospital through clinical quality and excellence.
- Reduces medication burden where appropriate.
- Initiates end-of-life discussions where appropriate.
- Collaborates with facility administrator and director of nursing to ensure operational metrics aligned to achieve goals.
- Provides consultations at the request of community physicians and community or Care Guide Partners nurse practitioners and making medically-necessary visits (billable) including telehealth visits when needed.
- Educates patients and their families about age-related health conditions and how to manage them.
- Serves as expert to organization on geriatric care needs. P rovides geriatric focused education to Care Guide Partners staff through informal consultations and educational offerings.
- Provides geriatric focused outreach for Care Guide Partners services to physicians and other healthcare providers.
- Makes direct contact and performs service recovery with physicians who have complaints or misunderstandings.
- Performs peer review of attending physicians’ utilization of services.
- Participates in development of clinical protocols and patient care guidelines.
- Maintains necessary hospital staff privileges.
- Maintains provider status with third party payors.
- Maintains accurate medical records and completes them in a timely manner.
- Provides consulting and attending patient care services, including medically necessary visits with appropriate documentation, coding and billing capture, medical orders, prescriptions, family meetings, and coordination of care with facility staff and Care Guide Partners or community physicians.
- Participates in on-call rotations for urgent orders, prescriptions, and telephone consultations.
- Participates in admission, recertification, pre-authorization, and utilization review decisions when needed.
- Participates in educating medical students, residents, and fellows.
Position Qualifications:
- M.D. or D.O. required.
- Licensure (or eligibility for licensure) for medical practice in KY, including DEA and controlled substance permits, required.
- Fellowship in Geriatrics required.
- Board certification or Board-eligible with expectation to be certified within 5 years of employment required.
