Education Requirements:
- Completion of four years of medical school.
- Successful completion of an American Board Medical Specialties approved residency program.
Licensure Requirements:
- Unrestricted Illinois MD/DO license.
- Illinois Controlled Substance License.
- Federal DEA.
Certification Requirements:
- Must be Board eligible upon hire.
- Board certified within the timeframe of the practitioner s primary specialty.
- Current CPR Certification (responsible for maintaining certification).
