Job Overview
- The Medical Home Medical Assistant provides both direct patient care in a primary care office and collaborates with care delivery providers to enhance patient outcomes. Key responsibilities include:
- Identifying gaps in care, contacting patients to schedule required care, and providing referral follow-up.
- Conducting pre-visit planning for the practice's patient panel.
- Coordinating messages through electronic portals and managing transitions of care.
- Acting as a clinical liaison for the physician care plan and communicating with patients.
- Participating in process improvements and understanding clinical goals, including patient satisfaction and engagement.
- Performing additional job-related duties to support the department's needs.
Job Requirements
- Graduate of an approved technical, professional, or vocational program in Healthcare; clinical experience in a physician practice or related field is preferred.
- Equivalent experience may be accepted in lieu of a degree.
- Certified or registered medical assistant; must maintain certification/registration status (RMA through AMT, NCCT, or NHA, or CMA by AAMA) and CPR certification.
- Strong clinical care, customer service, communication, and teamwork skills.
- Knowledge of EMR, practice management software, and medical coding/billing is strongly encouraged.
- Ability to make quick decisions based on well-thought-out consequences/results.
Job Responsibilities
- Demonstrate proficiency in the rooming process for adult and/or pediatric primary care, including clinical procedures, immunizations, venipuncture, point of care testing, and more.
- Follow protocols and policies for scheduling, clinical procedures, and appropriate use of medical equipment.
- Provide accurate and complete documentation of clinical calls and patient rooming info, order entry, prescription management, and workflow edits.
- Address patient messages (phone, MyChart) in a timely manner and escalate issues as appropriate.
- Maintain knowledge of population health, wellness, prevention, and chronic disease management.
- Participate in tactics to close gaps in care, including pre-visit planning, daily huddles, and informing physicians of potential barriers to care.
- Understand the adult and/or pediatric preventive medicine schedule, manage wellness checks, vitals and labs, immunizations, screenings, and patient education.
- Collaborate with wrap-around services such as behavioral health, complex care nursing, social workers, and community health workers.
- Provide basic community resources to patients with social determinants of health and support education and coaching for wellness and chronic disease management.
- Participate in continued education and training opportunities.
