What does a Transitions at Home RN Case Manager do?
Transitions At Home Registered Nurse Case Managers provide skilled nursing care to include assessments, medications, treatments, and coordinates services, counsels the patient and family/significant others in their place of residence in accordance with the patient’s physician orders and plan of care under the direction and supervision of the RN Clinical Manager.

Hours:

20-29 hours per week 8:00 am - 4:30pm
Monday through Friday
No Night Shifts (also known as PM and NOC Shifts)
Limited on-call rotation

Qualifications / Job Requirements:

Active Wisconsin Registered Nurse License
Two (2) years’ nursing experience, 1 year in Home Health (preferred)
CPR Certification
A valid Wisconsin Driver's License and Car Insurance
