Job Overview:
We are seeking a passionate and skilled RN Case Manager to join our growing team. As an RN Case Manager, you will play a crucial role in coordinating and providing top-quality care to our clients. You will oversee client care plans, supervise home care services, and act as a liaison between clients, their families, and healthcare providers to ensure the highest standard of care.

Key Responsibilities:

Assess and develop personalized care plans based on client needs.
Administer and oversee infusions in the home, ensuring the safe and effective delivery of medication.
Monitor and evaluate patient progress and adjust care plans as needed.
Provide leadership, support, and guidance to home health aides and nursing staff.
Ensure compliance with all state and federal healthcare regulations.
Collaborate with physicians and other healthcare professionals to coordinate client care.
Maintain accurate and timely documentation in accordance with BrightStar Care standards.

Qualifications:

Current and valid RN license in the state of Nebraska.
Experience in home health, hospice, or a related field preferred.
Experience with administering infusions in the home setting is a plus.
Strong clinical skills and knowledge of nursing practices.
Excellent communication and organizational skills.
Ability to work independently and manage multiple client cases.
