Job Responsibilities:

Provide specific case management services to individuals in residential programs.
Coordinate with community resources needed (medical, psychological, vocational, recreational, and supported services) based on the assessed needs and goals of the individual.
Assist person supported in becoming eligible for financial and other resources, which will enhance their independence and quality of life.
Complete Representative Payee Reports for Social Security benefits on an annual or as needed basis.
Monitor Social Security, Medicare, and Medicaid benefits to ensure benefits stay on line and services are not disrupted.
Maintain persons supported books and records, according to DDA and OHCQ regulations.
Review individual services on a minimum quarterly basis or as needed.
Prepare social summaries, and other necessary paperwork in conjunction with the annual PCP and in accordance with current DDA regulations.
Assist families, staff, and others in understanding the changing needs of the individual. Act as an advocate for the person supported and safeguard their individual rights.
Intervene in daily crisis situations to assist person supported in working through time-limited problems; as well as ensuring the most appropriate services are received.
Assist with coordination of transportation for person supported between programs.
Attend regular meetings, including Core Review, Annual PCP’s, Case Management, and committee meetings.
Utilize agency electronic systems for communication, progress notes, incident report filing, etc.
Attend and actively participate in all scheduled meetings.
Complete all state and agency required documentation in accordance with stated guidelines and deadlines.
Check and respond to emails daily to maintain proper communication.
Other duties may be assigned by the Director of Residential Services or designee.

Education:

Bachelor’s degree in psychology, sociology, social work, human services, or other related field and 2+ years’ experience with individuals with disabilities preferred. Associates Degree will be considered with substantial professional experience.

Training and Experience:

Preferred DDA core competencies, Mandt Systems Training, CPR / First Aid, MTTP. Experience working with people with IDD/DD preferred but not required.

Physical Requirements:

Person must be able to walk, stand, and sit for up to 8 hours. Person must have the ability to lift up to 50 lbs. or more based on assigned supported person (using health and safety lifting / turning techniques or assistive devices). Person must be able to successfully demonstrate all Mandt Systems training physical interventions and techniques, to include assisting supported person to standing, providing assistance with ambulation, physical releases, and physical restraints.
